Apostille for Medical and Vaccination Records

A visa officer, a university abroad or a foreign employer wants your medical certificate, immunization record or fitness to work letter apostilled. Health records are private documents, so the route runs through the provider, a notary and then the Commonwealth. Here is how to get each type of record into a form a foreign office will accept.

  • Provider certification and notarization arranged
  • Confidential, in-office handling
  • Rush timelines for visa appointments
  • Trusted since 2010
The principle

A health record is a private document, so a notary has to come first

The Commonwealth apostilles signatures it has on file: notaries, clerks, court officers and vital records registrars. Your physician, the hospital’s medical records department and the pharmacy that gave you a vaccine are not on that list. A medical letter cannot go straight to One Ashburton Place, no matter how official the letterhead looks.

The fix is the same one used for school records. The person who created or holds the record signs a certification in the presence of a Massachusetts notary, or you sign a copy certification for a record you hold, and the notary’s signature is what the Commonwealth apostilles. The foreign office reads the chain: the state vouches for the notary, the notary vouches for the doctor, the doctor vouches for the record.

Because the record is about your health, two additional things matter. The provider needs your written authorization to release it, and the record has to be complete and specific enough to satisfy the office that asked for it. Consulates refuse a vague “patient is in good health” letter far more often than they question an apostille.

The apostille does not certify that you are healthy. It certifies the notary. The doctor’s words are what the visa officer reads.

Record by record

The medical documents foreign offices ask for

Each one is prepared a little differently. The common thread is a signature the notary can witness.

Visas

Medical certificate of good health

A physician’s letter on letterhead stating that you were examined on a date, are free of specified communicable diseases and are fit to travel, study or work. Required for residency in Spain, Portugal, Italy, Mexico and much of Latin America and Asia. The consulate’s wording requirements are exact. Get them before the appointment.

Study abroad

Immunization records

Universities, exchange programs and some countries want a full vaccination history. In Massachusetts, the official record comes from the MIIS, the state immunization registry, through the My Vax Records portal, or from your provider. Either version is certified before a notary.

Travel

Yellow fever and other travel vaccine certificates

The International Certificate of Vaccination, the yellow booklet, is issued by an authorized vaccination center. It is recognized on its own by most border officers, but a few countries ask for an apostilled copy for residency files.

Employment

Fitness to work and drug test results

Employers in the Gulf, Asia and offshore industries want a physician’s fitness letter and sometimes laboratory results, apostilled or legalized. The lab report is certified by the lab director or by copy certification.

Legal

Medical records for courts and insurers abroad

Personal injury claims, disability pensions and custody cases abroad request hospital records. The medical records department certifies the copy as complete and the custodian signs before a notary.

Family

Disability and care documentation

Foreign schools, benefit offices and guardianship courts want diagnoses, therapy plans and physician statements apostilled, usually with a certified translation.

Step by step

How a medical record apostille works in Massachusetts

1. Get the receiving office’s requirements in writing

Consulates publish their medical certificate wording. Spain’s residency visas, for example, want a statement that the applicant does not suffer from any of the diseases that might have serious public health repercussions under the International Health Regulations of 2005, dated within three months. Portugal, Mexico and others have their own text. A letter that says something else, no matter how thorough, is refused. Bring the exact wording to the physician.

2. Have the provider sign in front of a notary

The physician prints the certificate on letterhead, signs it in the presence of a Massachusetts notary, and the notary attaches an acknowledgment or a jurat. Many practices have a notary on staff. When they do not, we send a mobile notary to the office, which is usually faster than asking the doctor to visit a bank. The notary’s commission must be current and the certificate complete. Our notarization guide lists the elements.

3. Or certify a copy of a record you already hold

For an immunization printout, a lab report or a hospital discharge summary, you can present the original to a notary and sign a copy certification. Massachusetts notaries are permitted to certify copies of private documents, and a health record is one. Some receiving offices accept this. Others insist on the provider’s own signature. We ask the office before choosing.

4. The Commonwealth apostilles the notary’s signature

The notarized record goes to the Commissions Section. The state charges $6 per apostille, walk-ins get up to three documents the same day and mailed requests take two to three weeks. We make the trip.

5. Translation

Spanish, Portuguese, Italian and most Latin American consulates want a certified translation of the certificate and the apostille, prepared after the apostille is attached. Medical terminology needs a translator who handles it regularly. We use ones who do.

Massachusetts specifics

Where Massachusetts health records come from

The MIIS and My Vax Records

The Massachusetts Immunization Information System is the state registry that providers report to. Residents can view and download their record through the My Vax Records portal, and providers can print an official MIIS record on request. Foreign universities generally accept either, once it is certified before a notary. If a vaccine is missing from the registry, the provider who gave it can add it or issue a separate letter.

Hospital medical records departments

Mass General Brigham, Beth Israel Lahey, UMass Memorial and the other systems release records through their health information management offices with a signed authorization. Ask for a certified copy signed by the custodian, and ask whether the custodian can sign in front of a notary. Some hospitals do this routinely for legal cases. Others will only mail a sealed copy, in which case you certify the copy yourself.

Pharmacies and travel clinics

Vaccines given at CVS, Walgreens or a travel clinic appear in the MIIS, and the pharmacy can print a vaccination record. Yellow fever certificates come only from authorized centers, several of which are in the Boston area.

Federal and military health records

Records from the VA, military treatment facilities and federal employee health units are federal documents. They are authenticated by the U.S. Department of State, not by Massachusetts. Our military records guide covers that route.

Privacy and timing

Two things to know before you order

Health records are confidential. We handle them in our office, never outsource them and return them by tracked courier. The only people who see the content are the notary, our specialist who checks the notarial certificate and the clerk at the Commonwealth who looks only at the notary’s signature. Translators sign confidentiality agreements.

Medical certificates age fast. Most consulates want the certificate dated within three months of the visa appointment, and some within one month. Schedule the physician visit after you have the appointment date, and order the apostille immediately after the examination. Our validity guide explains how the windows work across a whole file, because the medical certificate is usually the document with the shortest one.

Four medical files and how each one got accepted

Details changed. The problems are the ones our specialists solve every week.

Medical certificate · Spanish non lucrative visa

The doctor wrote the wrong sentence

A retiree in Andover brought a certificate that said “patient is in excellent health.” The Boston consulate rejected it because the wording did not reference the 2005 International Health Regulations. The physician issued a second letter using the consulate’s exact text, signed it in front of our mobile notary at the practice, and the apostille was attached two days later. The certificate was dated inside the three month window and the visa was granted.

Immunization record · University in Milan

The registry was missing one vaccine

A student in Cambridge downloaded her MIIS record through My Vax Records and found the meningitis vaccine she received at a travel clinic was missing. The clinic added the dose to the registry within a week, she reprinted the record, signed a copy certification before a notary and the apostille followed. The university wanted the translation sworn before an Italian court, which the family arranged in Milan after receiving the apostilled record.

Hospital records · Injury claim in Brazil

The hospital would only mail a sealed copy

A client in Fall River needed emergency room records for a claim against a Brazilian insurer. The hospital’s records office would release a certified copy by mail but would not send a custodian to a notary. The client certified the copy himself before a Massachusetts notary, the Commonwealth apostilled the notary’s signature and a tradutor juramentado in São Paulo produced the sworn translation. The insurer accepted the file. The Brazil guide covers the sworn translator requirement.

Fitness to work · Offshore employer in Qatar

Qatar is not a Hague country

An engineer in Quincy needed a physician’s fitness letter and lab results for a Qatari work permit. Qatar does not accept apostilles. The letter was notarized, certified by the Commonwealth, authenticated by the U.S. Department of State and legalized by the Qatari embassy in Washington. Four steps instead of one. Our guide to apostille vs legalization explains how to tell which route your destination needs.

What the four cases share

None of them failed at the Commonwealth. Each one turned on the step before: the wording, the completeness of the record, the willingness of a custodian to sign, or the destination’s treaty status. Get the receiving office’s requirements in writing, check the record against them before anyone signs, and confirm whether the country is a Hague member before ordering. The apostille itself is the easy part.

Timing the medical certificate inside a larger file

A residency application usually contains an FBI check, a passport copy, proof of income and a medical certificate. The medical certificate has the shortest validity window, often three months and sometimes one. Order the FBI check and the passport copy first, because they take longest, then schedule the physician visit so the certificate is fresh on the appointment date. The passport copy guide covers the piece most people forget.

Medical record questions

Frequently asked questions

Can a doctor’s letter be apostilled?

Yes, once the doctor signs it in front of a Massachusetts notary. The Commonwealth apostilles the notary’s signature. A letter signed without a notary cannot be apostilled.

Can I apostille my immunization record from My Vax Records?

Yes. Print the record, present it to a notary and sign a copy certification, or ask your provider to certify it in front of a notary. Either version is then apostilled. Some universities prefer the provider’s signature.

What must a medical certificate for a Spanish visa say?

Spanish consulates want a physician’s statement, dated within three months, that the applicant does not suffer from any disease that might have serious public health repercussions under the 2005 International Health Regulations. Use the consulate’s exact wording. Other countries have their own text.

Does the apostille prove I am healthy?

No. It proves the notary’s signature is genuine. The consulate relies on the physician’s statement, which is why the wording and the license number matter.

Can hospital records be apostilled?

Yes. The medical records custodian certifies the copy and signs before a notary, or you certify the copy yourself. Provide a signed release to the hospital first.

How recent must the certificate be?

Most consulates want it within three months of the appointment, and some within one month. Schedule the examination after you have the appointment date and order the apostille immediately.

Are VA or military medical records apostilled by Massachusetts?

No. They are federal documents authenticated by the U.S. Department of State. We handle that route as well.

Do I need a translation?

For Spain, Portugal, Italy, Latin America and most non English destinations, yes. The translation covers the certificate, the notarial certificate and the apostille, and it is prepared after the apostille is attached by a translator who handles medical terminology.
